Depends on the school. Most are supposed to wear them to class.
Most schools you don't wear them to lectures (most people wear normal street clothes, although this varies some between schools). For clinical rotations a good part of the time you'll wear the coat.

I think you get them for your 3rd and 4th years in med school because of rotations, correct me if I'm wrong. But, I now for sure it's not just a ceremonial thing, you wear it around campus etc.
Nah it varies.. ms1 we wore it like once a week to the hospital to interview patients or whenever we were doing clinical skills. to actual lecture i wore sweatpants and a white t prob everyday. one of the schools i interviewed at (nova) had its students wear scrubs to class and they were all color labeled based on if you were in the med, dental, pt or w.e school.. so it varies in that respect. i also hear lecom makes students wear ties to class? i think that all sucks and when u get to the point of applying/lucky enough to choose id highly recommend taking into consideration dress code and also attendence requirement.
You wear them during 3rd and 4th year during hospital rotations (unless your attending says otherwise) along with a nametag and identifier. The "short coat" is a pretty universal marker of a medical student in the US and in many other countries (Canada, the UK, India, etc).
During lectures with a few exceptions you can wear whatever the hell you want as long as it falls within an appropriate standard.

we have to wear them whenever we see a patient...whether for shadowing a physician the first two years, practicing clinical skills on standardized patients, or now in 3rd and 4th year pretty much year-round.
