I am not so sure about the cortisol decreasing insulin sensitivity, but that may be...

My understanding is this:

Cortisol is gluconeogenic, leading to hyperglycemia by breaking down muscle amino acids, hence thinner extremities. The hyperglycemia is what leads to hyperinsulinemia. Insulin preferentially distributes the new fat centrally. This is just my take though...
