How important is it to be familiar with the more "esoteric" molecular geometries like "distorted tetrahedral" and "square pyramidal"? The Kaplan book doesn't really address them so I'm assuming they aren't particularly important but I just wanted a second opinion.

I wouldn't sweat the details. You should be familiar with tetrahedral and trigonal planar and what hybrid orbitals get you there but other than that, I would say it's a low-yield topic. Useful to know for metal-based chemistry but the odds of that coming up on the MCAT are probably pretty slim.
