Mph after mbbs

How useful is MPH from US after doing mbbs from Pakistan/India in order to secure a residency in US? Does it make a significant difference or is it just sort of adding one more year to the number of years from graduation without enhancing your chances of getting into a good residency program?

If it allows you to add US clinical work to your resume, it's helpful. If it fills a year that you're going to need to take anyway (for the steps, for example), then it's helpful. If you're ready to apply for a residency, I would just apply. If you don't get a spot, using it as a filler year is reasonable.
