MTM provisions in HCR

Hello,

I have heard repeatedly that there are substantial MTM provisions included in both the House and the Senate drafts of the Health care reform bill. However, I cannot find much specific information as to what is included in the bill regarding MTM. Can anybody post links to such information?

Basically requirements for MTM in transitions of care, expansion of integrated care models (drs, pharmacists, other healthcare professionals collaborating together on LTC patients, a "medical home"), as well as provisions for grants to study the best MTM models.
 
The key is none of the previous MTM legislation nor the current versions specify MTM has to be done by a Pharmacist. We think of MTM as our domain but it can be done by any healthcare provider. Nurses are much cheaper than Pharmacists.
 
I see. Can any licensed pharmacist offer for hire MTM services to patients, or is there a separate training/certification process required? I am aware that a pharmacist who in not in a partnership with a physician could not make dosage adjustments or medication changes, but to provide a general medication review and medication recommendations, can any pharmacist provide such services?

I don't see why they can't, but they often don't get reimbursed for this. What is the motivation for a person to provide a service with no pay?

i thought MTM services have fees and medicare covers them. am i wrong?

Yeah, but nurses are cheaper. And what about the people who aren't on medicare? Still, MTM services are growing, though very slowly.
