I have a friend (who is Puerto Rican) that goes to school down there. She said that the tests and books are all in English, but all of the teaching is done in Spanish. And no, they do not provide you with language courses to help you out. The schools in PR are meant to be what our state schools are (they just don't have such a high degree of competition). They are considered US schools, they just cater more to Puerto Ricans much like many of our state schools cater to residents.

Does P.R. require fluency at time of matriculation?
When I looked into a PR school, it required fluency for acceptance. You interview in Spanish.
Do they provide language instruction?
I doubt it. Do you know of any stateside medical schools that would offer to teach a P.R. student English?

I believe that there are only two P.R. schools that take stateside folks. And I haven't met anyone that made it to the interview that didn't have a pretty compelling reason to head to P.R. Don't be lured in by the lower than average MCAT...

Materials and books are in english but most lectures are given in spanish (as sunnyskies above me stated). I have a friend who attends in ponce and she says this is how it is run...thankfully she happens to be fluent. I know another guy who is there who speaks NOTHING of spanish, so I have no clue how he is winging it. My fluent friend states that some lectures are even given in somewhat informal language (like using puerto rican slang terms for things and structures even, not correct dictionary spanish!), which strikes me as horribly unprofessional, but whatever...I guess you get what you pay for.
