Are you not allowed to get a B- in any undergraduate coursework?? Is that what it means when it says "A GPA of 3.0 on upper division coursework. Overall GPA and prerequisite coursework are required."?

I got hit with that exact same thought once. Not a pretty moment when I remembered that class that had been all MATLAB. Then I remembered that GPA = Grade Point Average. The average is taken over multiple observations, so it's not a floor regarding what a single observation can be. At least that's how I read it. If the standards were that high, most of us would be screwed.
 
I think it means you shouldn't have a C in 300- and 400- series courses. Even then, I doubt that a C in Underwater Basketweaving 401 is going to hurt you... But a C in Physiology 401 definitely will.
 
I know at my university for the major upper division classes like anatomy, physiology, pathology, stats we are not allowed to have a B- only B or better it all depends on the college. Next admission cycle i hear at my school they are changing it so that you can't have more than 2 C's in any of PT pre-req courses so it definitely seems like it's getting even more competitive.

For Puget Sound, you must have B's or higher (no B-) in your prerequisite coursework.

You need to inquire with the schools that you will be applying to and find out exactly what they mean by having a 3.0 GPA.
