N95 fit test and certification?

Hey guys,

So I'm applying for 4th year rotations and one program is asking for mask fit certification. Is that the same thing as the fit test? I just have a card saying the model of the mask and my size. Is this enough or is there some kind of training/certificate thing I'm also supposed to have?

I would just contact the program and ask/tell them this. I have never received a paper or certificate after getting fitted (or a card for that matter).
