I think you have to sign up online. Go into the dental professionals area on ada.org. It's right there.

i have already searched ada.org

in this link http://ada.org/prof/ed/testing/natboard/part1.asp i found pdf document called candidate guide which suposed to have exam application but i didn't find the application

so please any one have a copy of it please send it to me

Once u go to ada.org search NBDE and once the part1 page opens the online application is there under Supporting documents and apply online.

I've never heard of someone not applying online. You might be able to, but I don't understand why you would want to. You obviously have access to a computer. What's the problem?

i tried to pay online but i have a problem with my credit card ,i must ask the bank for suplementry internet card to pay online and this will take time
