I am developing software that will allow a dentist or other practitioner to record visits for reference/legal reasons. Hopefully the interface should be self-explanatory, and any feedback would be wonderful.

The interface doesn't look bad. It is appealing and easy on the eyes.

I like the fact that it is not very busy in apperance...kind of like the google site and that has made them pretty successful.

One thing though, I notice that you can only display about 6 names per page. If I have 2000+ patients, I would like to see more than 6 at a time to speed up the process of looking for them...unless there is some really good search function. I know you have one, but I'm not sure at how well it works. For instance I might have 40 John Smith's and I have to scroll through 6 names at a time.
 
Thank you for your feedback. I completely forgot about privacy laws that may get in the way, so I revised the initial window a bit:

 
Usually pts are searched by either number or last name.

Also I noticed you only have a one line entry per visit, usually we write more than that. If you entering a crown delivery you want to enter what cement was used, if anesthesia was used and how much and what type, etc. So per visit is more of a paragraph than a single line.

I wish there was a program that will have pre formulated entries so one doesn't forget stuff, like
Anesth used: type, amount, epinephrine?, topical prior to?

Based placed, type:
Post op intructions:
Prescription:

You know the typical stuff you write over an over. Specific pre formulated entries for standard procedures. If you choose crown deliver the computer asks you the typical steps you do during a crown delivery. The same for removable, endo, ext.

The reason I say this is because number of times we can't bill insurance companies for a surgical extraction because we forgot to mention in our entry the bone removal and what we used to remove it, etc. Or forgot to mention we used nitrous oxide and then we can't bill it.
