Hi everybody.
Im a medico from hyderabad India. Im in my 3rd year (i.e. final year part I), studying ENT, OPHTHAL and Social and preventive medicine.
It is my ambition to become a successful cardivascular surgeon after finishing with MBBS.

As you might know, here in India after MBBS we have to do House surgeonship for about one year. After that the degree of MBBS is awarded. After that most of them appear for ANDHRA PRADESH POST GRADUATION EXAM. As there are not many PG seats here, those who got a good rank they take up all the good streams such as Cardi, Neuro, Gyaenobs etc.. the rest they join diploma courses or some of them prepare for APPG again while the rest they prepare for USMLE or PLAB.

I want your valuable suggestion regarding what should be done after MBBS to become a cardiovascular surgeon. I want a map of what exactly i would have to do after MBBS. For example will I have to become a Civil Surgeon first and then cardiovascular surgeon? Or is there a course directly after MBBS that I have to join to become a CVS surgeon?

I am interested in studying abroad being a Permanant Resident of The USA. I would prefer to study in Australia or Europe.

So, can anyone guide me with a map to my destination? Any good colleges in Aus or Europe that anyone knows of? What is the procedure of admission in those colleges.

Any comments would be really appreciated.

Assuming you are not a citizen of the U.S., you will have to get licensed to practice in the U.S. That means you must pass your USMLE. Instead of worrying about what type of surgeon you want to be, I think it would be more valuable for you to start studying now to get the score you will need to even think about getting into that path.

To do "CVS" surgery, you would have to get a high enough (90+) score on your USMLE and apply for a General Surgery Program. It is unlikely that you will match with a score below that and its not easy to obtain that score. The APPGE exam is nowhere near as difficult as the USMLE nor is the PLAB. Assuming you are done with your 3rd year, you should take atleast 1 - 2 years off/work your ass off for the next 2 years and study so you can get a good score on USMLE.

Assuming your backup option is a PG seat in India, my advice is to go to the nearest bank, withdraw about 1 crore, and put it in a briefcase. Then I advise you to go to any private college and apply for a surgery seat as they will be more than willing to take you😉

So, I would need a 90+ overall on USMLE? Is that what you wanted to say? Then I should join the surgery program? Do u know approx. time it would take me to become a CVS surgeon in the US?

Well, my dad payed enough donation for my MBBS seat and I cant take a bank loan for APPG.

One more question. I completed my high school from the USA, so is it necessary for me to appear for the TOEFL or IELTS for my further studies in US?

Yeah, Im not a citizen but a Green Card Hold.

Yes you will need a 90+ score on your USMLE. Approximate time would be 5 years General Surgery and then another 2-3 years for Cardiothoracic fellowship/super specialization. It is very difficult to get into though and you should note that a back up option is necessary for the specific type of field you are looking at.

If your dad paid enough donation, what is another lump sum for PG seat? Might as well do it in India as you will get invaluable experience.

I don't think you need to appear for TOEFL but then again, if you don't have citizenship, you might have to. Either way, it shouldn't be a problem as your english isn't that bad.
 
A categorical spot in General Surgery is hard to come by. For every non US MD/DO that is. That may include FMGs or even US-IMGs. Not only would you need absolutely stellar scores, you'd also need alot of luck, good wishes and some sort of clerkship experience in US hospitals aka USCE. Currently, more than step 1 score, your clerkship grades matter when it comes to landing a residency spot.

Given you are in your 3rd prof, i'd suggest you concentrate on passing your MBBS first. The "house surgeonship" you refer to is a fancy word for internship which is mandatory before being awarded the MBBS degree. Internship duties are hardly taxing. Plan things so that you apply for clerkships as soon as you begin your internship. Get as much USCE as possible. Furthermore, nail the step 1 and 2 and hope to pass the CS in 1 go. Once you have all things in order, you should worry about matching into GS. Currently that should be the least of your worries.

Also, you don't need to give any english language proficiency exams. Step 2 Clinical Skills takes care of that.

That is a ludicrous comparison. The All India Post Graduate Entrance Examination is on a completely different pattern. In terms of duration it may be shorter but bear in mind that the AIPGEE covers all years of MBBS in one go. Where as the step 1 has only the first 2 years (+behavioural sciences) and the step 2 has the last 2 years of study of MBBS.

That said, you need to have a thorough grasp of basic sciences to ace Step 1. Where as in the AIPGEE, you may just get a handful of questions from the first 2 years' subjects.

Lastly, if you aren't a US MD/DO graduate, most (if not all) competitive residency spots will be out of reach for you. But once you get a good rank in AIPGEE you can get anything from Orthopaedics to Radiology.
