neurofibromatosis? not sure...

Regarding this old question that I found somewhere else...

A 35 yo woman, has had twelce 0.5-1-5 cm cafe au lait marks in her trunk, face, limbs for the past 5 years. what is the most likely diagnosis?
chronic hepatic disease
cushing's sd
HIV infect
melatonin excess
NF1


I was wondering if anyone could explain it to me..people said that it was due to NF1 but regarding the diagnosis criteria I don't really agree.. When do the cafe au lait spots appear? usually early on life right??if they are due to a fail in neural crest migration... melanocites... they shouldn't appear at middle aged... as well as the size should be >15mm ..please help!! 🙂

It doesn't fit the NF1 diagnostic criteria.
The correct answer is Melatonin excess.

It doesn't fit the NF1 diagnostic criteria.
The correct answer is Melatonin excess.

Melatonin? As in the compound that has nothing to do with pigmentation but everything w/circadian rhythms?

It's a great example of the real test like questions where they will try to use your memorization against you.
That's why it's important to answer the question they are asking rather than the question you want to answer.
 
6 or more cafe au laits, Lisch nodules, bony lesions, optic glioma, 2 or more neurofibromas (benign growths of schwannomas), 1st deg relative w/ NF1, intertriginous freckles <strike>How many needed for criteria? At least 2 beyond the cafe au lait?</strike>

nevermind i answered my own question. it is 2 or more of the listed criteria

it was from a really old exam, NBME 1, i found the exam somewhere and it was a pic of the question, so i guess it should have been difficult to change it rigth?
maybe it was an errata or maybe it s sth related to the chronic hepatic disease, as dfib slim said..
It could be some kind of cloasma-melasma like ? (I know that cloasma appears in the face)
due to decrease clearence of strogens? maybe this is the physiopathology of it, I just made it up.. . I mean I ve never heard about it, but could it be a quite reasonable answer..?

Could they be asking about Neurofibromatosis type 6 ? aka neurofibromatosis type 1-like syndrome

Hey sorry for the late answer. But I believe its NF1. I know this because I was born with NF1
