C sounds good to me.

"near miss" probably something like a mislabeled specimen or incorrect order that gets fixed before it affects the patient.
 
@seminoma Hey man.. please could you suggest a good resource to prep for these questions.. UFAP is deficient in this..and while these questions may seem obvious to many...i for one struggle with em..
also.... how many questions would u say fell in this category for u ?
