NICA for Florida License

What is this? Do I as an Internal Medicine doctor have to be Participating.

It says if i select non participation my fees are 1170 but if i do participating my fees are 5170

So I decided to google what the heck NICA was.... and the first thing that pops up is a result from the actual Florida board stating the exact circumstances in which you don't have to pay it:

That's kinda screwed up that they're making all physicians basically pay a tax (in the form of increased license fees) to cover what sounds like cerebral palsy, but what do I know?
