TBR has a few of theses scattered throughout.

Is it worth knowing the ones that are semi-related to AAMC topics (e.g., reactions of nitriles to form XXXX)?

where XXXX = AAMC topics like amines, ketones, etc.

Now that I have taken 5 AAMC's I can say that TBR does a GREAT job in preparing you for the orgo section. They introduce you to soooo much. Most of it you DO NOT need to memorize. The concepts you need to understand, but the orgo passages will give more than enough for you to apply those concepts to specific reaction mechanisms.

Most general nitrile reactions will not be focused on by the AAMC. To reiterate what @sillyjoe mentioned, TBR content does a great job prepping you for what organic chemistry topics you are supposed to know for the MCAT and, equally important, uses an appropriate amount of detail.
However, the best source to check this concern is with the AAMC content outlines. The one for Biological Science can be found at:

Do keep in mind that these content outlines list ALL the concepts that can be tested, but it does not indicate which ones are higher yield concepts than others. There is a list on SDN that provides opinions on what the higher yield concepts are, which can be beneficial, but it's important that you decipher that information through extensive question + passage practice as well.
