There are programs that give you more freedom like a PhD in epidemiology or a social science. If you want to do something other than basic science research you should look carefully and find programs that 1) Allow for this and 2) have strong research/a track record for MD/PhDs in the area you want to study.
That being said a majority of MD/PhDs are doing basic science research.

Yes, you need to pick the right program for you. For myself, I'm looking into Population Health Studies as my number one. My number two is Exercise Science, and my number three is Business, funny enough.
