Hypothetically speaking..

Say I'm a jr nsgy resident, completed internship and several neuroradiology rotations. If I'm interested in transferring in to radiology, how would you recommend doing so?

Easiest would be making connections within home program and them taking you in match or creating a spot. Getting a letter from someone you worked with would be essential for applying out.

Next easiest would be applying for some of the openings that come open around now from people changing their mind during internship or newly expanded spots. Often posted on Auntminnie or AUR website.

Lastly would be the whole dog and pony show of the match again, but you'd potentially have more say in location and quality.

Smart move if you make the switch. You can still have a ton of neuro involvement with neuroradiology. And if you feel like working ridiculous hours, you can do neuroIR... then realize that was crazy and switch back to diagnostic radiology later in your career. Good luck.

Ha, this wasn't for me.. But, can I get any years of my advanced training applied? I'm a PGY-5.5 now in the last sprint to finish. I've done my IR experience, done my reading "neurorads" as well as stuff that isn't related to you guys. Could I start as an R-2, 3-3? Say I wanted to neuro IR? I did my cases to knock a year off the nsgy fellowship..

If you think about IR or NeuroIR or something like that, stick with your residency and finish it.

I apologize if I am wrong. Honestly I doubt that your post is genuine unless you are tired of seeing patients and OR and want to do diagnostics.

I have no idea if you'd get any credit towards completing radiology residency, considering neurorads and INR make up probably less than 20% of total rotations...

Not only would you not get credit as an R1, but you may run into gme funding issues if you start your R1 year as a pgy-6. Someone Correct me if I'm wrong, but I believe you are not eligible for any more funding than the 7 years you were allotted when you matched in neurosurgery

Why not just do a fellowship in endovascular neurosurgery? (Would require finishing neurosurgery first.)

Would allow you to do diagnostic + interventional neuro rads.

If you do NeuroIR as a neurologist or neurosurgeon, you can not do diagnostics.

Hm, didn't know that. Figured the 1 year they get of diagnostic neurorads would qualify them.

To the OP: with only 1.5 years to go I really see no logical reason to switch! You're almost there. If you hate the OR, consider doing a neuro IR fellowship. Definitely better hours and schedule than most other neurosurg specialties. If you also hate neuro IR and really just feel like doing neuro dx, then I'd still finish neurosurg and then try to find a radiology spot. Keep in mind that life is always greener. A bunch of neuroradiology fellows in my institution are having real trouble finding jobs. A lot are doing second fellowships and others I know of have finished fellowship without a job. For those lucky enough to find a job, pay is very low and location not ideal. It's a a total different story in neurosurg. There are great job opportunities for those coming out of residency.
