Easiest would be making connections within home program and them taking you in match or creating a spot. Getting a letter from someone you worked with would be essential for applying out.Hypothetically speaking..
Say I'm a jr nsgy resident, completed internship and several neuroradiology rotations. If I'm interested in transferring in to radiology, how would you recommend doing so?

Smart move if you make the switch. You can still have a ton of neuro involvement with neuroradiology. And if you feel like working ridiculous hours, you can do neuroIR... then realize that was crazy and switch back to diagnostic radiology later in your career. Good luck.
If you do NeuroIR as a neurologist or neurosurgeon, you can not do diagnostics.
