NSU's secondary ????? eval form for DO?

    Hi,
    I got NSU's secondary today and I see that there is a form enclosed to send to a DO to be filled out and sent back to the school. For those of you that have already applied - is this IT for the DO recommendation or if we already have a letter can we use that instead? My DO recommender is far away and I don't know her ALL that well (well, I don't know that she'd remember me well enough to fill out this very detailed evaluation form). Do they absolutely need this form or would a letter be sufficient as a replacement? I will call Nova on Monday, but any thoughts in the meantime? Thanks! :) Liz :(

    If you already have a letter, you can use it instead.

    The way I recall doing it last year, there were a couple of Evaluation Forms to give to those writing you a letter of recommendation. Most of the schools I applied to, including Nova, did not require the form to be filled out. They offer it as a way to help the letter writers submit a recommendation, but they usually actually prefer the letter. The form is a supplementary, non-mandatory item that can be used in addition to the letter, or even instead of it.

    I did NOT send this form to my osteopathic physician that wrote my letter. I just forwarded the generic letter my D.O. had written for all admissions committees because he wasn't as close by anymore. I wanted to complete my file quickly. However, I did ask other recommenders (a M.D. and a Ph.D.) to fill out Nova's evaluation form since I was still currently in contact with them. Was it necessary? No. Your application will still be considered complete without the form, as long as you have the letter.

    I would still call Nova, however, just to make sure things are the same this year. I don't recall a D.O. specific evaluation form that was distinct from the other evaluation forms.

    Just so you know the possible time frame, I received my secondary at the end of August, submitted the secondary at the end of September, and received a letter to interview at the end of November. Good luck!

    I second what Dagny said.... I personally used a committee file/letter, but when I called them last year they simply told me to turn in the letter.... The evaluation is there to help your letter writer if necessary.

    You might want to try and give them a call, it's better to hear from the horses mouth.

    Thank you both for the info - that's quite a relief (that I shouldn't have to track my DO rec. down to get her to fill out the form). I will confirm this with Nova on Monday, but thank you for easing my mind. :)

    I agree with POPOY...I too did the same thing as he did...and just submitted my committe letter....I interviewed and was accepted at Nova....so it wasn't a problem at all...

    I ultimately declined....and chose Western/COMP, but I did the same for all my other secondary applications..and I had no problems.
