What tips/ suggestions would you offer for handling nursing malignancy? It seems in my department lately there is no winning. The kill them with kindness approach seems to be going nowhere. The nurses are openly insubordinate, question orders and treatment plans in front of patients/ families, and many times refuse to address doctors in the manner which they are entitled to be addressed (aka Dr) 😡

Other than generally knowing how to get along with people, there's nothing you can do. They don't work for you. Open you're own office, get out from under the oppression of EMTALA and hire your own staff. All of a sudden, when you're writing the check, you're a King (or Queen).

Nurses have bosses. You're best bet is to address this with the nurse manager (possibly through your director). It's possible they are the root of the problem, but they may also be ignorant of what's going on.
 
I'm not saying I endorse this neccessarily, but I watched a guy use brutal pimping in front of patients to fix such problems....

"Oh maybe you're right, what do you think her Wells score should be?" Nurse: Uhhhhhhh.....

"I see your concern. Remind me again, what is the pathophys of such and such a disease?" Nurse: Uhhhhhhhh.....

"Well here, show me what you're seeing on the CT scan?" Nurse: Uhhhhhhhh....

Not the way to make friends though.

i'd take it up with your director first, and coworkers... are you the only one with these issues?

i've had such issues only once out of many hospitals between residency and beyond... turned out it was a pattern w/ younger female attendings. i moved on... luckily i am part of a multi-site group. everyone won.
