I work in an urgent care and the other day a regular nurse told the NP, hey when you have nothing to do, take a chart of the pts that are waiting and get their vitals and put them in a room. It was not well taken by the NP, she answered "do the doctor have to do that" ? the other nurse responded, " no but you are a nurse like us. The NP response was she will write a complaint against the other nurse.
They feel superior and feels need to be treated like a doctor.

I'd by lying if I said that things like this aren't somewhat steering me away from this whole primary care thing. I'd like to think that a good doc could never be replaced, even with all this craziness going on, but I don't know... one has to wonder what the scene will look like 20 years down the road. McNoctors all over the place on every corner?

Maybe I'm just having a pessimistic weekend or something... but this stuff kind of bums me out!

Surprisingly enough more and more of NPs and PAs are going into specialties. No physician can just close their eyes and pretend encroachment isn't happening to them.

I am aware of this trend, however something tells me NS is safe.

One of the NP's that floats at an Urgent care I moonlight at....her full time gig is Neurosurgery NP.


I can tell you that as a 3rd year resident, it's scary watching how bad some of these NP's are...I've got plenty of examples.

I don't doubt it. I'm not worried about NPs competing with me in the future, I am however worried about the idiots in Washington putting a bandaid on this physician shortage problem, which we all know is coming. Going into neurosurgery was a joke....kind of.

I agree. It's not NP competition per se. It's the government stepping in and screwing things up even more (e.g. support of using NP's as autonomous practitioners in the name of this flawed new system they wish to implement). I honestly do not believe the government will have a solution for the physician maldistribution issues though, short of providing more loan forgiveness programs with less headaches. With budget issues consistently on the agenda, I wouldn't expect much there. Also in terms of opening the flood gates with more GME positions, IMO, no use saturating already already soaked areas that will be chock-full of noctors in the coming years.

Let's face it, our government is a mess and most of what they touch turns to...

Of course I wasn't there when this happened, so I don't know all the details about their exchange. But it seems quite plausible that this little tiff could have had just as much to do with the RN feeling "inferior" and it did with the NP feeling "superior".

I'm just a 2nd year resident, but work in a large system with many PAs and NPs. I've lost count of the number of times I've seen patients mismanaged and/or misdiagnosed by them. To the profound detriment of the patients. Not to mention the number of ROUTINE cases that any FM doc could handle that they refer off to the specialists. They do not make the system cheaper or more efficient, and certainly not safer.

I've seen my share of mismanaged patient's by NP's. Unfortunately many patients I've seen prefer a female provider over the actual knowledge of the NP/PA they r seeing and view us all as having the same knowledge.. I had a gal I saw in urgent care with end stage cirrhosis and the only med she was on was atenolol!!! No lab monitoring, no diuretics, no pain managment, no KCl, no nothing. She came in for "belly pain" ended up having ascites, hypotension, etc. Lab results showed K of 1.7 (not the only abnormal lab) - she was emergently admitted, I informed her NP down the hall that I was taking this patient off her panel and got her in with an FP to establish better care. Was not the first time I have taken patient's away from NP/PA's who are too complicated for their knowledge set.
