As an upcoming May 2014 grad, I am a confused on the paperwork required to take the June 2014 compounding exam.

Do I have to submit proof of 1,040 intern hours through Form 4 to the state board? The rules state that, "this form is not required for PharmD graduates from ACPE or CCAPP accredited programs." Does any grad from last year remember whether they submitted form 4?

If you're graduating in May 2014 and taking the exam in June of 2014, you do not need to submit Form 4. Form 4 is only required if you are taking the exam early during the end of your P3 or during your P4 year.
