observation/volunteer hours

I had a question about doing hours for observation and volunteering. Exactly how should I be recording this? Are the schools going to ask me to show any type of proof, or will they just go off my word? I'm planning on doing a volunteer program at UCLA to boost my hours. I just don't want to be surprised when I'm asked to show proof and I don't have any!!
 
Some of the schools require that you get a letter from the dentist you were observing as proof that you put in the hours. Look at the requirements for Tufts for example.

The thing that I am wondering is if this should be just a straight forward "X observed for Y hrs." with a signature from the dentist or it should be a recommendation.

It can be either/or. If you want to use your dentist as a reference, then you can include a recommendation along with the statement of spending X amount of hours in the clinic.

On the other hand, if you want to use your dentist just for observation purposes then yes, "X observed for Y hrs" is fine. Just make sure that you ask your dentist to include what kind of procedures you witnessed.
