Hello everybody,

I received an occupational heath form to fill from my residency program. It requires information about immunization dates and the titers for MMR and hepatitis b in addition to PPD test. I was immunized for MMR last yr and for hepatitis in medical school.
I am a FMG, I just moved to the States and I have no family pysician here. My question is: Where can I fill such form? Are these tests covered by insurance?

You should be able to get the info from your school about the immunizations you got during med school, and any others that were reported to them. If not you should be able to get the necessary titers drawn and the PPD placed with no problem at emloyee health (if you are working in a hospital right now) or a health clinic. These tests may be covered by insurance, but you will need to call the insurance company to find out.
 
I couldn't find some of my records so I went to the local public health clinic. It is cheaper than going to a physician's office. They take insurance for some things as well so it still might be cheaper for you if your insurance doesn't cover all of it.
BTW, the PPD was $7, the Hep B series is $135 and the follow up CXR and TB meds are almost free.
In the past I got a MMR from there because I couldn't find my old records and it was less than $10. They do physicals as well for pretty cheap.
 
I doubt it'll all be covered by insurance. I've had to have similar tests done before med school, away rotations in fourth year, etc. They weren't covered because they weren't considered "scheduled" or "routine."
