Office Visit checklist

Hi, I'm a recent graduate now helping my FP preceptor redesign his charting/dictation process.

I was wondering if anybody out there had a copy of a successful checklist (fast but thorough) that they use for office visits. There have been some ideas in Family Practice Management magazine, but I am more interested in things that people have put into practice and found useful.
