Mainly went by opportunities in a field outside but not separated from radiology. I only applied to locations I wanted to live, so that wasn't that much of an influence. I wanted to be in a smaller city.

IR, location, gestalt came into play. Honestly, it's all bunk after the first 6.
 
Damn, I had high hopes that I would have a good shot at my number one, UVA. After seeing the rank lists here, not so much.

I'm guessing you're talking about MUSC? If so, I can tell you why I didn't rank it higher...

It started at the beginning of the day, when the PD asked all of the interviewees why they decided to come to the interview. I explained that I would like to stay in the southeast region, MUSC has a great reputation, and I currently have an interest in IR and I've heard very good things about their department. She started on a terror about how I was going to be "out of luck" with doing an IR fellowship because the DR/IR dual certificate and she's on the committee blah blah blah with a very arrogant attitude. I just said, "well, if that's true, I'm sure I can find something else I'd be happy with in Radiology." Mind you, I had just spoke with Dr. Angle earlier that week (someone who *IS* on the committee) and he said that basically we don't know what's going to happen just yet, but fellowship will definitely still be an option. Also, I had talked with another attending in the summer about it and she said the same exact thing. Okay, there's the first part, just like how no one wants to work with an arrogant trainee, I don't want to work with an arrogant PD. The funny part about it is that some of the applicants snickered after that, but then about 1/4 of the other applicants said virtually the same thing. Many of them said, "I'm John Smith and I want to do IR, and I heard it's really good here." I think she lost a significant audience after that silly spiel.

The next part was the fact that I didn't learn anything during the interview. They presented us with very little actual information about the program.

Finally, they danced around my questions like a seasoned politician. "Do you have any questions for me?" "Yeah, I hate asking this question, but what are the weaknesses of the department in regards to resident training?" "Oh, so far we've hired Dr. X, Y, and Z to improve our previously lacking department X." "Okay, that's great, but what's weak right now?" "Well, as you know, we're in the midst of a change in the testing format, so we're not really sure what we're weak in right now. We'll have to reassess after we get the results back." "Okay, so you don't know what you're weak in as far as testing, what about clinically?" "Oh, we feel those go hand in hand. We've got an amazing cardiac imaging department, though!" Me: -_-

The major positives were the residents and the city. Everything else was sketchy, IMO.
 
LOL at the dancing around questions. That happened to me at other Southern programs. I then had flashbacks about growing up in the south and realized I am too old to deal with that nonsense anymore, so I dropped all my southern programs far on my rank list or opted to not rank them at all.

MUSC in my top 3. Did not get this vibe at all. I found the PD to be direct and the faculty to be pleasant. Imagine being 20 years into the profession in their shoes and you have some medical students walk in and profess a delusional amount of love for a specialty in which they know next to nothing about. They then start questioning you about your program's weaknesses. I would do the same thing I do whenever narcissistic IVY leaguers tell me how great their new Tesla > infinity magnet is: yawn.

I'll tell you what I tell everyone else that questions my interest in IR. I did a month solid of IR - put in lines, did lung biopsies, thoracocentesis, chest tube placements, drained misc abscesses, did bone biopsies, did paras, and even did a pancreatic FNA all as the primary operator. I've been the only other person scrubbed in ablations, fistulagrams, angioplasties, biliary drain placements, PCNs. How many studies have you dictated? I've done 0. I THINK I'll like reading studies... I KNOW I like doing procedures.

I also had just spoken to two people intimately involved in this accreditation process, so it's not some ***** medical student speaking, it's two people that WILL be making the decision about the certificate. Not some PD that *thinks* she knows what's going on.

I did not ask about the programs weaknesses in front of everyone, I only did it in the one on one interview. It's also a legit question that shows you that the program is introspective and willing to admit faults. I don't want to go somewhere that won't discuss weaknesses with people that want to come there as a resident.

Fair enough I guess. I didn't get any of that when I interviewed there and I know the program pretty well.

And you still ranked them in your top 5...

Very true. I did. I kept them there because I felt that the quality of the program was high enough based on what other Attendings have told me, the amazingly awesome residents, and location. I feel that it's a tremendous program, and a place that I would be okay with (might grow to love), but not my first choice. Obviously you didn't get that vibe, so I hope you end up there if that's where you want to be. Honestly, I feel that some programs just don't match up well for some people... I'm sure you felt that at a few of your interviews.

I agree with the above. Sometimes, although a program may be a wonderful program overall, it just lacks some things that you desire in training. I'm sure the programs all say the same thing about us applicants as well.

Sounds like your experience was a negative one, but I didn't get that impression at all. I go to school in the SE, but I'm not from the region. I've done a good number of cases as primary and first assist like you at multiple institutions and I liked MUSC for their IR focus. I didn't hear anything negative re: IR-centric applicants in the Fall when I interviewed. I'm not sure what could've changed so considerably.

I enjoyed the campus, the IR focus, the strength of their cardio program, most of my interviewers, some of the residents. I did not like the lack of jobs for my SO in the otherwise wonderful location. In the end, that's what did them in on my rank list.

I loved Charleston and I really liked the people at MUSC. Unfortunately for me, they just didn't seem that strong in neuro, my area of interest. I was hoping that the acquisition of the fMRI group from NYU would be an upside, but I didn't get the impression that many neurorads collaborated with the neuroscientists in that group.

Not to sound like a weirdo, but if your app has the caliber to get interviews at MGH, Hopkins, UCLA, UW, etc...unless you're couples matching...why would you go on 19 interviews?

Do you apply and interview separate for the Direct Pathways? I guess some programs have separate ERAS programs listed?

It was an additional program in the application process. It was the same interview, but this might change given the new DR/IR path. I'm not sure how programs will handle the switch. Most likely they won't make you interview separate.
