Awesome post Don!!! The only SOAP note I know how to write is for athletic training. Doesn't exactly lend itself well to the hospital.

CER sucks. The class makes me hate life. I was at home on Thursday for a funeral (great grandpa couldn't take the nursing home I guess), and we got 11 inches and big winds that day at home. All the schools in the area cancelled for both Thurs and Fri. So emailed both the CER email and Dr. Mitchell Thurs night to see if I had to make it back for CER Friday afternoon. Guess when I got a response ... last night. 😡 And turns out I wouldn't have had to go. Most of the drive wasn't bad, so I was just 25 minutes late. Grrrrrrrrrrrr.

I have to go to my 3rd clinical H&P tomorrow. I hate it. I'm not getting anything out of it except how little of the physical exam I have memorized, which I don't give a **** about until the 3rd week in April. And the coordinator set me up with a different doctor for the last 3 visits, so I feel like I'm starting over.

Anyone else notice that Leslie seems to be back? I'm confused. I thought she quit?

Some scut is running around drawing blood cultures, calling the lab because they decided they didn't feel like running the BMP on your patient even though you really need Na and Cr levels to do your FeNa equation prior to rounds and then you have to explain that the lab sucks, calling people's weird family members, etc.

Some of it is actually scut, like copying pages or looking up articles or getting dinner. But it's mostly the stuff at the beginning--stuff that you could learn from. At least, in my experience, that's been the case.

Incidentally, I should be working on a presentation right now but I got called in to Kaplan tonight so I just got home twenty minutes ago. I am NOT going to be bright-eyed and bushy-tailed tomorrow. No sir.

My 200 lb. TV died today after only 16 months (with a year warranty, of course). Now I need to drag the thing down to the dumpster and go buy another overpriced monstrosity.

Unfortunately my wife is set on a budget that is about $200 less than the Sony I want to pick up (another huge 200 lb. tube TV beast) so I'm sitting here contemplating whether I want to wait around for prices to (potentially) drop or start hawking off my old-school video game systems and accessories to make up the difference. Life is complimicated.

Yours truly it is. He was really nice too - he came over to my place (he lives in New Berlin, I live a mile north of school in Tosa) just to help me carry it up my stairs. Like Funk, it's a 180 lb. tube, and there's no way my wife and I could have done it. He and I were straining at a few points on the stairs. My wife and I carried the couch up our stairs.....let's just say that next time I'm gonna pay the $60 for delivery.

Looking up information (phone numbers, etc.) in old medical records on patients you've never seen, just so the resident can clean out her "to do" list on the electronic medical record (EMR).

Well, they know the ABC thing but the way the question was worded made some people second guess themselves and now they're pissed.

It asked something like, "If a person is in hem. shock what is the first thing to do?"
A. Circulation
B. Ventilation
C. Airway
D. Wallet biopsy


I figured from the answer choices that he was referring to when he said "you always secure an airway first" or something along those lines. But, the distractor kind of sets the situation to be as if you've been working on the person for a while and you've figured out that they are in hem. shock. If you overthink the question like that then you're going to put A.

So, if you reword the question:

"if a person goes into shock in front of you, what do you do first?"

or

"if a person goes into hem. shock what do you need to do to promote compensation?"


Does he want to know if I know what is causing the shock or does he want to know if I know what the ABCs are?

This is probably a case of me over-thinking a question, but here was my reasoning:

If a person is severely bleeding, ie. in hemorrhagic shock, isn't the first and most logical step to stop the bleeding? Assuming you are the only responder to an emergency, you're not going to save the patient by checking their airway if in the meantime they're bleeding profusely. Once they lose too much blood (as you're stablizing the airway and breathing) the shock becomes progressive and irreversible. So, I chose A, circulation, since it's the closest option to 'stop the bleeding'!

You can tell s/he was never in EMS, because ABC!!!! If they're laying in a pool of blood, you might want to see if they're even alive first. Bandaging someone with rigor mortis wouldn't make a lot of sense.
