So we're making the Peds Sub-I this month take one day of weekend call. The visiting student and I are handling the rest.

Also, you will have the option (and it really is optional, not "optional") of spending a week at the VA or Froedtert to get some exposure to adult uro if you so desire. The cool thing here for you would be to spend some time with Dr. Kressin, who does a lot of female urology and female sexual dysfunction. Her lecture is also a do not miss. Entertaining and informative.
 
didn't you work a lot harder for those better grades though? I still can't believe someone in your class got a 99% on the pharm final.

How do you find out all this stuff? Do you sit behind bookcases and listen to conversations, or what?

Well, the grade spread was posted in the Hall of Shame.

Prowler also saw that I practically lived in the library, so yes, I worked harder for the better grades, but I still had more fun. No, I didn't get the 99%, close to it, but not it.

And weekend call is just rounding, with the option of "we'll call you if there's something interesting". The only weekend day they may have a surgery outside of a true emergency is Saturday, and those are usually the "can't wait until Monday" cases.

Spending a week at Froedtert would really be up to you. It might be fun to throw in a little variety toward the end of your month. Plus, you attend the same lectures and morning conferences as we do, so you'll hear about a lot of adult uro topics, so it may be fun to actually see some. Dr. Kressin said the other day that she's only had one student in her clinic since she's been an attending, so she'd probably be thrilled.
But you have to take what I say with a grain of salt, because I love the stuff. A day in the OR or the cysto suite may not seem as fun to you. But you'd see a broader scope of urology if you spent a week on the dark side.

Question for you current M3s. What is Pathology-Question the Experts that occurs Monday afternoons? Trying to arrange a little trip back out West and am curious if it is a required attendance type class? Thanks!

If I recall correctly it was mostly just an "office hours" type of session with that week's lecturers where students could ask questions to clarify the lecture topics. Definitely wasn't required attendance last year.

Weren't they annoyed that no one showed up to those? That was the rumor, and they were going to make block 2 hard because of it, but I never went.

well, they definitely succeeded. i didn't learn the difference between half of those leukemias and lymphomas until a week before boards. oh, and of course I didn't have ANY questions on them.
 
Xandie, where does one get scrubs at the Chitlin's Hospital? they failed to mention that to us, or at least to me. i plan on acquiring some prior to my next shift.
 
The only place I've ever thieved scrubs from is the OR. It's on the third floor and your ID should get you in. However, that's a great question for whoever you're working with on your next shift. Show up like 20-30 minutes earlier and just sheepishly ask one of the nurses or interns... that's how I roll. 😛
 
My 4th of July shift in the ED was scheduled as such: Noon until 10pm.

The following conversation took place between my attending and I around 2:30pm.

"finish up the discharge orders for that patient and go home."

"you mean I should go home? I'm supposed to be on until 10pm."

"when I tell you go home you go home." He then mentioned something about how it was going to be dead in there.

I'm really not sure how to take that...but I definitely got some extra shopping and studying in! Plus I made myself a cheeseburger. and had a really nasty beer. Now I'm stuck with the other 5 because they aren't that great but I wanted to try it. Anyone want a free nasty beer? come on over and we'll hang.
 
by the way...is anybody else totally digging Jackson Browne's solo acoustic album? I know it's gotta be rocking at least 3 or 4 of your worlds out there too. if not, it should be.

I'm going to improv a story now using only a 2 word phrase I wrote recently on a post it note.



Samuel was a thin, sick man. The deep creases and dark emptiness on his face told lies of old age that only the year of his birth could confound. For years, he was the most important man at the most famous restaurant in three counties. He folded the tortellini. On his deathbed, he was often heard to say things like "I don't regret any of my choices," and "I can die in peace knowing I lived my life to the fullest."

But just as the appearance of his skeleton face told lies, so did he. Years of doughy, paste-crusted fingers, endless bowls of spinach, beef, and cheeses. This was what his life amounted to, and nothing more. The bold statements he told on his deathbed were a blanket to keep him safe from the truth which he could not accept for himself, nor reveal to others. He was ashamed of himself.

People said the tortellini at Spinuccio's was the best, because it was so carefully handmade, and each tortellini was stuffed with the optimal quantity of beef, cheese, or spinach. The flavor was said to explode in one's mouth with such intensity that you could feel the passion Samuel used to fold each individual one. But really, Samuel cared little for the work; it was all he had--he owed a significant financial debt to the owner of Spinuccio's that he could never repay even with a lifetime of work. The supposed passion came simply from the story printed on the front of each Spinuccio's menu: "the flavor of Italy is in each bite, placed there by the passion only an Italian could know."

Samuel was in fact not Italian; he was of mixed German and French lineage. Perhaps there was some Greek in there too, but it was only a rumor amongst his grandparents.

His debt to Mr. Bjorn, who refused to be addressed by any other title, came from a sizely loan he needed to woo Darlene, who died in a car accident shortly after a successful wooing. The money wasted, Samuel was left to repay a debt that forever reminded him of the women he loved and so quickly lost.

But now, close to death, his fingers too weak and lacking the dexterity to fold a single tortellini, he tried to pretend it was all worthwhile.

1. She directly told me her grades.
2. The grade spread for pharm was on the bulletin board next to our phys final.
 
Had a call yesterday with a relatively young female who was totally, totally out of it. She looked fairly pregnant, but we were informed that she had gotten her period on Monday. I mentioned to my partner that she could be spotting from a miscarriage as well. Anywho, we got her down to our ambulance, took a blood sugar, and got a number in the thirties. Well now, that's not our call anymore! A paramedic rig rolled in and took her away, but we ran into the same crew later, and they filled us in on what had happened (some of the info came from the hospital).

Turns out she was pregnant - about 10 months along or so. 😱 The baby was about full-term, but had died in utero, as much as a month ago. The mother was in surgery, but had a very poor prognosis on survival.

Well, the sepsis would explain why she felt hot to the touch, and the dead baby would explain the foul smell. 🙁
 
Oh, and I saw one of the brand new M3s at the VA yesterday. I didn't know her name, but after quickly browsing Asher's facebook friends just now, it was Amie.

dude, i'm on outpatient peds. i can go visit it whenever i want, practically.

And I'm on inpatient intensive care peds, which means I get to go to the bathroom once a day. Usually.

Dude, psych is busy. I've yet to get out early. Not like I should complain to any of you guys on the Big 3, but this whole "psychiatrists work 3 hours/day" is BS. At least my attending is really nice.
 
Just a tip for those of you who haven't done inpatient peds yet--know your nutrition and fluid management like the back of your hand. Those sick kiddos sure don't eat for themselves very much.

Also, just an update for those who care: my rotation so far has been pretty busy, even though I only have one patient right now and she is fairly stable and working on being discharged to home palliative care. The Intermediate ICU where I am has all sorts of zebra cases and most every kiddo there suffers from a major chronic disease that usually bleeds into many body systems and makes management really difficult. I am simply awed by how much the attendings and senior residents know about medicine. Makes me feel like an incredible idiot.

It is a feeling you'll come to know well as a JMS. Yeah, there is a lot to learn. But that's okay, just read. I can't emphasize that enough.

Also consider that the senior resident has four years on you, and the attendings have been doing this for much longer. Next year at this time, you'll be amazed at how much you've actually learned in one year.

I'm still surprised how much I've learned in 2 years. Like I can follow medical conversations without getting lost. A lot of times I have to look up drug names on Epocrates when they don't use generic names, but I like being able to understand the basics of what's going on.
 
Call last night was actually kinda fun in OB. Helped deliver 4 babies and assist in 2 C sections. Ob is nice cause everyone is usually pretty healthy
 
Who is the Mojito/ mint julep fan here? What is your recipe?

That would be me, I believe. Fresh mint is the key.

Mint Julep:

Put 4 or 5 mint leaves in the bottom of a double Old-fashioned glass or Julep tumbler, and add 1-2 tsp. of granulated sugar. Muddle (gently mash up with a wooden spoon if you don't have a muddler) the mint and sugar to release the oils from the mint leaves. Fill the glass with crushed ice and add 2 oz. of bourbon whiskey. Stir until the glass is frosted, then top up with more ice. Add a mint sprig for garnish, toss in a straw, and you're set. I like to use a ton of mint in the garnish, and I cut the straw short so you really have to bury your nose in the mint to drink it...you wind up getting a wonderful fresh minty aroma that really makes the drink.

Mojito:

Put 7 or 8 mint leaves in the bottom of a mixing glass and add 2 teaspoons of granulated sugar. Add just a splash of club soda and muddle until the sugar is dissolved and smells like spearmint gum. Add 2 ounces of light rum and 1 oz. of FRESH lime juice (squeezed from real limes, not that ****ty RealLime juice you can buy in the little plastic bottles). Shake with ice, and strain into a collins glass filled with crushed ice. Top up with club soda, and add a couple sprigs of mint for garnish. Some people like to muddle 3 or 4 lime wedges with the mint/sugar for their mojitos, but I think you're really just making a minty Caipirissima then.

Damn, now I'm thirsty. It was a good (hot) day to drink either of these today.
 
Psych at the VA is pretty fun so far. Busy, but we're seeing interesting, very psychotic patients. I would have gotten out a little early today, but it took me 2 hrs. to write up an H&P on an admission after taking 1.5 hours to get the H&P. I hope I get faster at this stuff soon!

Don, I have Dr. Tsao as my attending. She seems really nice, and definitely doesn't expect much out of us right now (spent 20 minutes teaching us how to write a prescription today).

What kind of beer are we talking about here? Is it skunky, or do you just not like the style?
 
So I was at the hospital for 15 hours. And I need to get 2 H&Ps written up for tomorrow morning. I just keep reminding myself that my intern has got another 15 hours on top of that. It was a good day though. I'm just overwhelmed by all the work that needs to be done once I get home. Haven't been able to get any reading done really. And feeling a little burnt cuz I'm used to sleeping 10 hours/night. But Saturday's my day off, so I hope to catch up on both reading and sleep.

So far, I'm very surprised about how much I don't hate internal medicine.

And I could go for a good mojito right now.
 

😱 I'm surprised this happened in an Edina golf club. Just proves the fact that the upper class are not immune to bad things happening.


I would gladly take beer off of anyone's hands if it's necessary.

And I am a hard-core night owl. Just that I'm not allowed right now. Driving home tonight I had to remind myself that it wasn't 3am and that's why there were so many cars on the road.
 
So I just found out now that Normandy is showing my apartment tomorrow at 3pm. My place is a disaster. I have empty boxes everywhere. I haven't been able to put my laundry away or unpack from last weekend. And I'm watching Gina's dog who may or may not have an "accident" while I'm gone cuz it's not his house. Plus, the dogs will not be in a crate. They'll be free in the bedroom. Even if I stay up all night cleaning the place up after getting my work done, I still don't think this showing will go all that well.

Now I'm not only tired, but irritated too.

That sucks. You should tell them to f-off and come back next weekend or they'll just be wasting their time showing the place to someone who won't want it after seeing the condition that it's in.

Great Lakes Brewing Co....called something like "holy moses" beer. supposedly has a spicy citrus taste with some other junk in it. I shoulda learned my lesson the LAST time i didn't like a Great Lakes beer.

Dead Guy Ale from Rogue....now THAT is a tasty beer. Same goes for Red Stripe.
 
by the time I'm done in the ER I won't even know how to do a full H&P. I love how abbreviated everything is. The sheet I have to fill out has a space for the HPI that is literally about 3/4 of an inch by 3 inches. diagnosis? hardly ever need one. EMS did bring in some kid that was probably no older than 12 that was totally plastered tonight. I bet his mom was thrilled.

oh, man, that totally reminds me of a time I had a kid just like that, but he might have been more like 14 (but no older). He polished off something like half a liter of E&J (barf) and smoked some weed, and he was just GOOOOOOONE.
