awesomest thing ever: got a page from someone asking if I wanted to go to lunch, but didn't bother to actually say who they were. they did sign it "M" but I'm too lazy to go on a search to figure out who it is. I can probably guess though.

I'm tired. I don't like psych. Today sucks thus far. I also just bought a book that was highly recommended to me by my dad who said it was a great read: The ABC of Acid-Base Chemistry: The Elements of Physiological Blood-Gas Chemistry for Medical Students and Physicians.

My dad also said Goodman and Gilman was one of the best written textbooks he ever read. My dad = nerd.

If you look up Don and Xandie in the paging directory, it gives their title as "Junior Medical Student."

If you look up any of the 09ers, it says "Medical Student."

classes of 08/09, how many people had to repeat a year that you know of? two of our class will be joining the class of 2011 that I know about.

I believe that only one or two of my classmates repeated first year, and I can think of four that repeated second year (although I heard it was seven, but can't figure out who those other three are).

And I'm pretty sure the only people celebrating are Gimlet and Ash, Stringbean, Splat, April and Amie have until the end of the month for their joyous celebration. (And yes, I realize that was a crazy combination of screen names and real names. Run with it.)

Donnyfuego, I forgot to tell you that I saw an adrenal rest the other day! It was interesting, and I thought of you. 🙂

April was there too. I celebrated by having a Chancery hot fudge sundae. those are good.

Wasserman likes to pimp you about sutures. Like which one you can eat (vicryl), and what they are made of (it's on the label).

He also points to areolar connective tissue as they do the dissection during c-sections and ask you what it is. It looks white and fluffy, and is in between the fascia and muscle. He also asked me the layers of the rectus sheath (know that below the line of douglas, there is no posterior sheath). Otherwise he's okay.

My preceptor in clinic was Dr. Fenske, and she's super nice. I never had a chance to work with her on L&D, but I think she would have let me do the delivery.

I'd avoid Koh, he's a jerk, and he doesn't let you do much. Janik doesn't let you do much in the OR, and is arrogant, but she doesn't pimp you. Hogerland I never worked with, but I hear that you should know the branches of the hypogastric (internal iliac) artery for him. He also pimps you in clinic, but I don't know what about. Boruta is pretty cool, and he'll let you do some suturing sometimes.

On L&D, the nurses will call you for any H&P, but for the most part will never call you for a delivery. You have to bird dog those closely. If you know the patient is 7-8 cm, I'd stick around. Otherwise, they are ok. Some are nice, some aren't, but if you treat them nice, they are good to you in return. As always, don't piss them off.

I also hated working for the midwives. They expected you to do the H&P, but then never let us help out with the delivery.

On the other hand, the OB residents were great, and let us do a lot. Also, if there is a FP resident they will often let you do quite a bit.

On slow days, Alterra is within walking distance down Prospect, as is Q-Doba. If you are more adventurous, you could cruise down to the Alterra by the lake. This is easier on Wednesday mornings when the residents are at lecture and there is no clinic.

The call room is ghetto, and I didn't get much sleep, but other students were able to get 5+. It's pretty easy to book out of there ASAP post call too. Another good place to hide is the library (at least during the day).

That's pretty much everything I can think of, in no particular order.

whoa, that reminds me, I watched him do several operations a few years ago when I was shadowing an anesthesiologist (AKA, landscape client) at St. Mary's. That's kind of a hard name to forget, haha.

Ashers, the neuro ICU says hi. They talk an awful lot about you and javad. Sima is awesome. How do i do an H&P/progess note on a pt that is intubated and in a coma?

Sweet. Do you mean how do you do a neuro exam on such a patient? We didn't write any notes or anything while we were there... no access to their system.

Yep, yep,you can do more. I probably sounded dumb because 1st I'd go in and yell at the person, then basically talk really loud anytime I was about to do anything.
Pain -- press on their fingernail with a pen, penlight or something hard. See what they do in response to pain -- localize, withdraw, extend, flex, nothing.
(II and II/III) Check both pupils for symmetry and reactivity to light
(III, IV, VI) Move head slowly -- only if neck's not broken or potentially broken whatever (then use cold calorics)
(V, VII) Corneal reflex -- touch cornea with a 2x2.
(IX, X) Technically you can check gag. I never did.

That's about all you can do. We had some lecture with a neuro test in a comatose person. I think it was like on the first day. Anyway, hope that made sense... just woke up from

I hope that made sense... just woke up from my 3 hour nap after getting home from call. I have to do some interviews in spanish by myself. Not fun... lots of Spanglish. This is also solidifying my idea that newborns = disgusting.

Oh, and apparently being in the NICU helped my presenting skills, cuz this morning one of the residents told me I did really well with presentations, and I definitely didn't learn that on psych.

Joe is with me there. They (sima and dr lynch) just referred to the way yall use to do things. And how javad wanted to know why rasmussan's only effects half of the brain and dr lynch made up some bogus answer about receptors
 
Tailgating in the rain is tough. But at least we went to the game last night, which the Brewers actually won.

My mom was supposed to be coming down this weekend. Then I get a call at noon Friday from her saying that both my grandmas were coming as well. It was just like injecting stress right into my veins. It was a fun weekend.

Xandie, Donnyfuego... any thoughts on vascular surgery at the VA? Also, what books are good for surgery since we don't do the shelf? Thanks so much you guys are awesome!

No idea on how vascular there is. Sorry.

I recommend the "underground" packet of answered study guide questions. They used to sell it at the MCW Kinkos. I'm not sure if the new place has it. If you don't have any luck there, let me know.

Haha. Lynch is awesome. I don't think you get to work with him much though during the next 2 weeks. Sima said something about Fitzsimmons and some other person. Fitzsimmons is really nice and really likes to teach. He did rounds with us 1 day.

I don't care how much sleep I got (9 hrs) getting up at 4am, I'm still way too tired.
 
Trust me, 4am in August is a million times better than 4am in January.

Oh yeah, and vascular is probably the most liked specialty service because the attendings are awesome (aka Dr Lewis) and the surgeries are pretty cool. I'm partial to CT, but that's because I'm a mega-nerd. Although personally, I think vascular at the 'Dert and general at the VA is a better track, but whatever, you get a month of VA surgery anyway, and that's pretty key.

IM assignments are in. Like always, they have you and me reversed regarding inpatient/outpatient. Will we EVER be together???

by the by....all moved into my new place. but probably won't have internet for at least a few more days. That may in fact help me in terms of studying for peds shelf.

have i mentioned I don't want to be done with peds?

Like Romeo and Juliet, perhaps keeping us apart is ultimately better for both of us. At least I have your supafly current teammate as the other JMS on my team and once again I have avoided gunners like they're the plague. We're gonna rawk that joint.

VAMC Team 1, hooooo! Anyone have any idea whatsoever what kind of call schedule that puts me at? I've got a wedding I'd like to make it to and need to know if that will be my golden weekend.
 
Team 1!! If there's any gorgeous handwriting on that board (from people's phone numbers), note my mad skills.

Umm... ask Amie, she should know. You could add it up all the way from me being on call March 3rd on team one. 🙂

I think it depends on your attending, but you should be able to pull it off.
 
I have a practice pelvic exam tomorrow on standardized patients (like in CER), so Mary's chief resident gave us the morning off!!!!!! YAY!!!! I don't have to wake up at 4am, but I bet I will anyway. 😀
