I would say good if:

1) you clearly write your purpose as a physician
2) you aren't picky where you go to medical school from this list (i.e., you are realistic)

We don't have your application, and your descriptions are not insightful to gauge these two conditions for me. I trust your advisors and letters are true champions for your application.

Your chances would be better if I could tell how much service orientation you have done before applying. Founding a club is leadership.
 
I have worked as a Patient Care Assistant (PCA)   for over 1500+ hours and built platform for Cancer patient outreach/support programs.

I am intersted to get into a MED school (not picky) and would prefer in state or colleges that can/will accept my credentials.
