OMM interning?

I'm going to a DO school next year (haven't decided which), and I plan to use OMM in whatever I decide to do (provided it's feasible...I'm thinking about doing OB/GYN or General Surgery). I know that to actually be able to use OMM, you have to do an extra residency or internship. How long does that take and can you specialize in something after you do it? Can you do an allopathic residency first, then do OMM? Do you have to do the OMM first? If so, can you do an allopathic residency afterwards?
I'm not sure if this is posted anywhere...I couldn't find it.
 
You don't have to do an OMM residency to use it in practice. An osteopathic internship would be helpful, as you will get more practice in OMM during your intern year, but even that isn't necessary. All you need to do OMM is to have a license (and have gone to a DO school, of course.)

From what I've understood though, to become proficient at OMT you really should do a residency or fellowship or something. Is this not true?

Use it regularly and you can become proficient. You don't necesarily need residency training.

No. You certainly can't do an AOA fellowship after an ACGME residency unless you do the internship year. There is no need for an OMM fellowship to do OMM. Any DO can do it.
