how are they different?

gastroschisis = abdominal contents directly in the amnion
omphalocele = abdominal contents are in a sac

In omphalocele, the intestines are covered by the umbilical cord. Usually the intestines herniates out in the umbilical cord because they grow more rapidly than the fetus, but then they return back in the 10th week, failure to do so causes omphalocele. Whereas in gastroschisis, it usually comes out of the left side of the abdomen (I don't know the reasoning behind this) and is due to a failure of the lateral body folds to fuse in the 4th week'

Side note
Failure of the caudal body folds = bladder extrophy
Failure of the rostral body folds = sternal malformations

gastroschisis is on the RIGHT side. it's due to ischemia of the right-sided abdominal wall when the right umbilical vein or right omphalomesenteric artery regress prematurely. it leads to a full thickness defect and the bowels end up floating freely outside the abdomen in the amniotic fluid (not covered by any sac). secondly, gastroschisis is a deformation; omphalocele is malformation (embryologically).

omphalocele is basically a hernia at the base of the umbilical cord (can contain bowel, liver) and is covered by a sac of amnion (unless the sac is ruptured, so this is not absolute)

this is a favorite pimp question in peds and peds surgery btw.
