Recently took care of a patient with TAXUS stent placement <1month ago. Presented to hospital with increasing SOB, found to have PE. Already on ASA 325mg and Plavix. We start patient on lovenox and transition to coumadin therapy. My question is what to do with her ASA and Plavix. Do you continue ASA, Plavix and coumadin. It seems to be alot of anticoagulation for a 79 year old with other multiple problems. Any recommendations out there?

Also, any benefit to long term anticoagulation with severe CHF, EF of ~10%?
Thanks.
