online HPSP forms?

Are any of these HPSP forms online? I want to e-mail the evaluation form to my reccommender but my recruiter only gave me paper copies of the forms.

[edit]: Are the rec forms supposed to ask about my current specialty etc? Some of the questions are not relevant since I haven't even started med school yet... I wonder if my recruiter gave me the wrong forms...

I'm applying for the Army HPSP. I am particularly looking for the "Applicant Evaluation Worksheet" which I am supposed to give to my reccommenders (Form 195-R-E). I tried googling it, but I couldn't find it.

Beyond that, I don't know. I'm just a Navy guy. 🙂
