the patient was seen 3 times in the ED. she signed out AMA on one of those visits. the job of the ED doc is to screen for life threatening conditions. she did not have a life threatening condition. it is the job of her GP to diagnose her condition. eventually she was sent for the US by the GP. I see no fault in her care.
