3. State of residence or country of citizenship (if non-US): NC
   
4. Ethnicity and/or race: ORM
   
5. Undergraduate institution or category: Elon
   
6. Clinical experience (volunteer and non-volunteer): 500 hours and 5 years of hospital volunteering at 2 hospitals in the ED
   
7. Research experience and productivity: 4 years, 1 publication, 1000 hours
   
8. Shadowing experience and specialties represented: 100 hours, multiple specialities
   
9. Non-clinical volunteering: 100 hours at food bank, 3.5 years
   
10. Other extracurricular activities (including athletics, military service, gap year activities, leadership, teaching, etc): 4 years managing hot dog stand booth as leadership

Any ideas to improve my extracurriculars? Also, I want to stay in state so I am only applying to Campbell, ECU, Wake Forest, UNC, at Duke. WAMC? Thanks guys!

You should receive some interviews from your state schools though that is too small a number of medical schools to apply to. Suggest adding a few more such as:
Virginia Commonwealth
Eastern Virginia
UVA
George Washington
