Ortho/Specialty Letters Needed?

Can anyone lay out approximately how many letters and from whom I should have letters from for ortho or other specialty programs?

Also, where should I ask these letters to be held?

I had a pretty performance in a few classes this semester and wanted to get the ball rolling on my app in case I decide to specialize. If I do, it seems ortho is the most likely route at this point.
