Ok, coming out of a DO school, one competes either against MD's for a residency or can go the full DO route to a DO hospital (eventhough, in both cases, a "DO" is the degree).

It's understandable that against MD's, it would be competitive for a DO to achieve a surgical sub-specialty.

But don't DO hospitals also need those same specialties? don't they need all specialties? I mean, if they don't, then that's understandable.

But if they do, why would it be tough for DO's to get one of those specialties? I would assume that if DO hospitals only let DO's in, and if those same hospitals need all sorts of specialties, then it would be as difficult for a DO to get a neurosurgical residency from a DO hospital, as an MD trying for the same from an MD hospital.

Someone, please explain what I am not understanding.

What has happened over the last few decades is a steep increase in DO schools and DO graduates. However, the number of DO residencies has stayed relatively the same. Therefore, with the increase, those subspecialty residencies are just as competitive.
