its this pdf file you have to print from their website, and basically it verifies you have shadowed at least 20 hours, and the dentist you shadowed has to check some boxes as to whether he thinks you are an excellent, good, fair, bad, or poor candidate for dental school. just another hoop to jump through. i do know they won't offer you an interview, or consider your app complete, until its in.
