I got an email from Ohio State Univ, they want me to complete their 2ndary application, there is also a shadowing form and can be printed of their website. my question is do I need to print it and have my dentist sign it? or the AADSAS "shadowing" section will do the job? I mean even though I listed whom I shadowed and the hours I shadowed on the AADSAS website, do I still need to to print the form and have my dentist sign and mail it to OSU??

it sounds like they want you to prove you shadowed a certain amount of hours so i would fill it out and mail it in.

The dentist doesn't need to sign it, but you need to fill it out or your app won't be complete. It asks for more info like contact info for the dentist, procedures you saw, and three things you learned while there. I don't think they will actually contact the dentist, I think they just want more conformation that you were actually there and learned something!

I was under the impression that it needed to be signed by the dentist you shadowed because there is a place for him/her to sign it.

if you have a letter from dentist in your AADSAS application maybe you don't need that. Just call and ask them. good luck my friend 👍

Umm i think u can submit multiple observation report if you've shawdowed more than one dentists...i remember reading that in the instructions pdf for the observation report..u can check that out on the osu website. GL!
