The OTCAS program will send an email to your references that will allow them to enter/attach a letter of recommendation, and also has a link for them to complete a rating form of sorts. (They definitely have to write a true letter of recommendation - one of my supervisors wrote her letter incorporating comments from OT's and PT's I worked with.)
