Hi. I have been wondering why do we, by default, implant leads in the RV instead of LV when someone needs a pacemaker. I haven't really found the answer online so I figured I would ask here.

Ease. Usually you use one of the upper extremity veins so you have access to the right heart.

To get a lead to pace the LV you need to stick it in the coronary sinus, place an epicardial lead surgically or theoretically you could get to it from an artery but youd never get a reasonable seal where the wire entered the artery. Furthermore, you'd still get the dissynchrony if you just paced the LV.
 
via the cs... or sticking the subclavian artery instead of vein during implantation...

I was trying to get them to think instead of doing their homework for them.
