Is anybody out there doing this? Is it viable financially?

I will be dual-boarded in geriatric medicine/palliative care and will work primarily in nursing homes. I would like to bring palliative care to that setting, but can't afford to spend the time needed for a proper consult if I am billing under traditional codes (I have to maintain production to justify my salary).

Can anybody offer some advice/insight?

I bill with time codes. It works out pretty well. I spend a fair bit of my time in nursing homes, and I love it. Try carrying a traditional black bag in with you; patients, families, and staff really get a kick out of that.
