It will now be one big section of questions rather than divided into 4 distinct parts. The level of difficulty isn't really supposed to increase, but you are also not able to perform a partial retest if you are weak in one or more areas.
The ADA website can provide you with more exact info. Check it out to get all of your questions answered.

The new exam could actually be easier cause with 20% of the questions being clinical, it takes some questions away from the more difficult Biochem and Microbiology questions that most people don't like.

It might not take questions away from them, but make them easier and more relevant. I personally find questions relating to clinics much easier than ones that have nothing to do with dentistry. Having a bunch of questions related to clinics will remove a small bunch from the rest which should make the exam slightly better. Of course this all depends on weather you are good at your clinical knowledge. I personally am a graduated Dentist so clinics is more fresh than studying for Biochem which i haven't done in 5 years!
