Hi everyone,

Recently, I've been looking for a few patient summary sheets to use on the wards and in the ED. Last year, I had several very good blank forms for medicine wards and a decent summary sheet for the ED, but lost all my information when my computer crashed. After several unsuccessful google and SDN searches, I decided to post something about it here.

I thought I would ask folks here if they had any links to good generic forms for use in the hospital or if they had created any that they would like to share here. I'm especially looking for things for use during my internal medicine months, a MICU month, and in the ED (but of course post anything that you feel would be helpful).

Got a set at home. I'll post 'em when I get there. Haven't used them , though. In the ED, I just put stickers on a piece of paper, leaving a little room to jot notes. On the floor and the medical ICU's, the med team had sign-out sheets that were standard, so just used those.
 
I would also be interested in a summary sheet. Also, if anyone has any tips on writing up patients, or preferrably a sample of an ED write-up, I would be greatly appreciative. I am a medical student in Israel, and as such, I do not write in the patients chart because I cannot write (or read, or for the most part, speak) in Hebrew. My first rotation in the states, in August, is a Sub-I in EM. I really do not want to look like an idiot when I cannot chart/write up patients correctly. Any assistance would be appreciated. Please PM...

then look under wards (i think) and it is broken down by speciality, but no EM...
These may be the same ones turtle has, but look them over and decide what is best for you.
As far as write ups in the ED...i have never seen anything formal at the few EDs i've been in-it's scratched notes of "OPQRST" and the pertinants.
I'd like to see something official if it exists.
Where are you doing your AI? I'm getting nervous about mine already-3 weeks and counting!

Of course, we were required to do "Write-ups" on our patients, but they were not part of the medical records and I am not sure how closely they approximate those required in the US. I have been trying to remember the ones on medfools. Thanks for the link. New York Presbyterian is my first one.
