Hi Everyone,

Congrats to us on submitting our applications. Now that they are available to the programs for review, please post your updates on this thread.

DO, mid tier program south, Step and COMLEX were in the average range (Gosh do these scores still matter?? They keep asking for it I guess). One publication in Residency (case report).

Honestly think that Step/COMLEX scores are pretty low-priority for fellowship interviews. I think the biggest piece honestly is going to be what your letters say and how you're able to convey your interest in the field in an interview :)

I have 4 invites so far at places I’m pretty interested in. I only applied to 17 programs with location bias to the west coast. Should I try contacting some places now? A little email to say hey I’m here? I know it’s only been a week.

No, please don't do that :) As you said it's only been a week, and as you can see here probably only a handful of places are sending invites at this point.

1. Seems that the west coast programs send invites much earlier than east coast programs. Anyone else seeing the same pattern? Any idea why?
2. If a program previously had two fellows per year, and now only one fellow is listed on their website for the most recent cycle - is that a red flag? Why would programs decrease their number of fellows for a given year?

For 1, it's probably just a coincidence TBH, or you're not seeing people who applied to east coast programs posting on SDN for whatever reasons.

For 2 I echo what the other poster said, programs haven't been filling lately. In light of this, a few programs have started downsizing as there is no point in budgeting for fellowship slots that may not fill.

In general I wouldn't expect rejections from most programs until near the end of the cycle. Unless you're a super-top tier program (like CHOP or Cinci, which are the two programs with R's in this thread) with no shortage of applicants, it just makes sense to have a large pool to pull from in case you need to fill a canceled interview slot at the last minute.

Worth a read. Looks like 32 programs (44% of all programs) not filling 43 positions last year (and the trend is worsening every year in the last 4-5 years). If you look at more granular data, even the programs with high reputations, such as Stanford (1/3 matched), UCSF (2/3), UCLA (0/2), Emory (3/6), Wash U at St. Louis (1/3), Duke (2/3), Cinci (4/5) and more, did not fill all their spots last year. It may be risky, but if you are truly unhappy with places you interview, you can decide not to rank them, and try to get a spot outside the match (assuming you have connections to find out the programs that went unmatched quickly after the Match day).

FWIW, I do agree with some of the conclusions. The increased use of APPs is, I think, leading to a decrease in the number of "strictly clinical" jobs in a lot of practices. I do also think that taking a 4th year (or more) of training to hone your niche is increasingly becoming requisite to get a faculty job. Alternately, you may wind up needing to stay on as faculty at your fellowship institution for a few years (if they're able to make that happen).

I strongly encourage current applicants to consider the track record of programs in ensuring additional protected time (ie a 4th years, sub-fellowships, etc) in making their decisions. While it's not fun to ponder spending MORE than 3 years in completing training, I do think that you need that additional time to be able to differentiate yourself from all the other graduating fellows. I also think that fellows should consider some of the other non-clinical jobs out there (pharma, FDA, etc)

No comment on the idea of trying to "scramble" into desirable programs, though theoretically it seems like a possibility :)

Just depends on the program, their funding, and in the case of fellows looking to stay and continue their bench research on the ability of the individual fellow to obtain external grant funding. There are a handful of highly-regarded programs which are able to guarantee a fourth year for everyone. There are also a number of programs which advertise specific non-ACGME accredited super-fellowships (ie BMT, neuro-onc, sarcoma, etc) which may accept applicants from external programs (though they often will give preference to their own fellows). Bottom line, I think it's something you should ask about in your interviews.

I would venture to say that the market for heme is generally a bit better, but also program and region-dependent. Remember, a bunch of programs are smaller and don't necessarily separate their heme and onc services, so for those kinds of practices you're fighting for the same jobs.
