Can anyone explain the malnutrition and dehydration?

Also, is it the bullae (blister) makes it vulnerable to the secondary bacterial infection?

Your skin protects against some insensible water losses and also acts as a barrier to infection. Denuded skin loses water and protein. Denuded skin also allows bacterial infection to set up. Same thing happens with burn victims.
 
I think with pemphigus vulgaris you can also get blisters in your mouth. Might make one less inclined to eat --> malnutrition/dehydration.
