Do you write a different personal statement for prelim & transition year applications from the one you wrote for in anesthesia? I've written my anesthesia one and it took a painfully long time to write the damn thing. I dread writing something totally different for prelim year. Have people been writing about their experiences in internal medicine for prelim personal statement?

I have the same question. One resident who I spoke with said that he only changed the last paragraph of his PS for his transitional year application. He had no advice for the Prelim year statement since he didn't apply.

Anyone with some advice?

No, you don't need separate statements or LORs for internship positions. I just added a paragraph at the end of my personal statement explaining why I wanted to do a transitional year. At the time I was applying, this was what other people I knew were doing too. The programs know that you are only there for one year before moving on to your chosen specialty.

I used the same PS as for my anesthesia programs, but changed the last line. I was never asked about it on interviews.

I did exactly the same as above.... added a few lines to the last paragraph about why I wanted to preliminary medicine.
