I apologize if this has been addressed already, I couldn't find much using the search function. I am trying to figure out if all D.O. grads must do a transition/traditional rotating year prior to starting their field of residency. Does it depend on the school you attended, the state you are in, or some other factor?

Not all DO grads have to do this. As far as I know it has nothing to do with your school. There are the five states that require that year but you can go through a formal request process to have your intern year counted instead. It also depends on what specialty you are going into because I know that at least some dermatology residencies require that additional year before being able to apply directly to the derm res. Someone correct me if I'm wrong please.

no, only some speicailties require these internships before you start in the residency as a PGY2

not at all related to school.

there are 5 states that require an osteopathic intern year (not a separate internship, though) or certain intern rotations for licensure. this does not mean that anyone will be doing an extra year of training over an MD residency...the internship is incorporated into the residency time.

Thank you for clearing that up! Do you happen to know which 5 states those are?
