BC is a 3.5kg 12 week old that was born at 30 weeks gestation on day 3 of vino-venous ECMO for severe pneumonia on Gentamicin 14mg IV and Ampicillin 265mg IV q6h. Gentamicin peak and trough levels ordered around the fourth dose infused from 0900 to 0937.

Peak: 5.7mg/L collected at 1019
Trough: 0.8mg/L collected at 0800

As the clinical pharmacist, what do you recommend...

Half life 7.7
Cmax 6.07
Min 0.73
Vd 2.53
Tau ?
MD Change dose to 21mg IV q24h infused over 30 minutes
