I was wondering if any of you have your patients sign a HIPAA form before you take photographs of their eyes/faces for your collection, presentations, posters, publications, etc.?

If so, perhaps you could post a sample form on this site so that others could use it as well.

If it's the eye and non-identifiable, then HIPAA doesn't really apply. Verbal confirmation is all I seek. If it will be identifiable, then I use something like this:
